Multiple pot holes in speed hump

There are multiple large potholes in the speed hump. The speed hump break height regulations and now there are pot hole hazards too. Please fix before a cyclist is unseated or you get a compensation claim for vehicle damage.

There are multiple potholes in a 2 square metre area. Two of the holes have had some asphalt put in them. All the others have been ignored. Very poor job.
