Pedestrian hazard - uneven road in several places

This has been reported previously and a repair was carried out however tarmac has and is breaking away, plus even more defects to road where pedestrians cross, a lot of elderly, pushchairs, walking aids etc and is a hazard. This is the entrance to store car parking, residential and other shops, however, the area has yellow lines which indicates the council responsibility.
