Drain cover does not sit flush 25mm above frame, the frame is already raised 3 inches above pavement

This is clearly uneven, and not easily identifiable, my son’s Partner was out running today and tripped over this, sustaining injury to both hands elbow and both knees. The fact that is raised so high I’m sure it’s more of a danger in low light too.
