Unsecured manhole cover - dangerous

The concrete holding the drain cover is disintegrated and making the cover loose plus dangerous if someone were to stand on it. Outside 3 Rack Ends driveway so cars have to pass over it so further damage will happen which is unavoidable. Only noticed today.
