Dangerous pavement - several falls - risk to life

There are large sections of pavement along Weston way in particular that have caused several people I know to fall badly over just the past 3 weeks alone. One of the incidents resulted in an A&E trip with stitches needed across her leg and was unable to work as a primary school teacher as a result.
2 weeks later I was out with a friend and she fell in almost exactly the same spot for the same reason. Luckily that time she (only) ripped her clothing and got away with a bruised grazed knee and a limp for a few days.
Most of the pavement along Weston way from church way to Abington park, particularly on the right side looking towards the park is in serious need of repair.
If I know 2 people personally that have fell in just the last few weeks, there must be many many others.
If nothing is done, the risk to people is off the scale.

Investigation: Completed (Defect Found), Defect Repair: Not Started - We have assessed the location and we can confirm that the defect meets the requirement for a Priority 3 repair. We will schedule a repair to be completed within 28 days.
