Uneven surface due to use of large vehicles

Please see the photo attached as there is a very big hole in the road that is leaving an uneven surface and it is causing a significant amount of noise from vehicles driving over it outside my house and it sounds like the cars are driving over icy patches. Please, please forward this photo and message onto the correct people or department so that it can be fixed as soon as possible as it is driving my into a worst state as I have a triple Mental Health disability including ADHD bipolar and traits of borderline personality disorder. I hope this is satisfactory description in order for this problem to be rectified. Thank you.
