Subsidence of road way in two places on opposite carriageways

The road surface just before... the house at the top of the hill from the junction of Shirville's Hill & Goring road, on the Cray's Pond direction, and, on the opposite side, in the Woodcote direction a section halfway between 2 telegraph/electricity poles.(Woodcote/Goring heath parish). The road has subsided so much that vehicles are crossing the central line to avoid them and this is on a blind corner at the top of a hill. We have also sustained damage to our passenger side front wheel rim, as one has to accelerate to get up the hill and was unable to avoid this depression, due to traffic. This is going to cost myself for to the lack of maintenance of this starch of road.

As of yesterday there has still been zero attempt to fix/make safe this issue. Cars are swerving all over the place putting vulnerable road users in extreme danger.
