Failing previous repair to a pothole/sinkhole

At the end of last year a significant repair was made to a very large and deep pothole. This repair is now failing quickly. Given the rapidity of failure it would suggest it is more than a pothole and that could be some sort of subsurface void. The pothole is in a challenging location and impacted by buses at the start and end of the school day. Quick investigation and response needed.

Tape measure is at 24 inches and is 4 inches deep Every Bus in and out of the school has to traverse this pothole because the road is narrow

Whilst a repair has been made to this pothole there are already significant signs of of the centre sinking. It is more than a simple pothole and requires a more detailed investigation into the cause.
