Pavement raised and broken over tree roots

I have previously (over a year ago) reported this issue with the roots of a tree lifting the pavement. It is getting worse as you can see from the cracks in the tarmac and the fallen paving slabs. I estimate the height of the hump caused by the roots is at least 12” and it is a significant trip hazard.
