Uneven surfaces on pavements

The surfaces on the pavements on Aldercombe Road are very uneven where they intersect with individual driveways. The pavements were resurfaced this year but the sections where residents drive over them from their properties are made of concrete and are cracked and very bumpy. It's very hard for some one in a wheelchair or with a pram to walk over them and particularly move down from the pavement to the road because the drop curb is not very low. The section outside our driveway (number 40) is very uneven to drive over. Does the council fix these places? Or is it the responsibility of the homeowner to make sure the pavement outside their driveway is maintained? The issue has been there for a few years but appears to be getting worse.
