Noisy and loose manhole cover

The manhole cover is loose and the road surface around it is uneven so everytime a vehicle drives over it there is a loud noise. This causes the residential buildings nearby to shudder. The manhole cover is on a traffic heavy road and is also in direct line with the busses so all the buses drive over it 24 hours a day
