Uneven paving slabs outside GP Surgery

Sent to Bristol City Council less than a minute later. FixMyStreet ref: 7784606.

Directly outside the GP Surgery there are some uneven paving slabs. This has gradually got worse over the past 6 months but it is causing a trip hazard on the entrance/exit to the GP Surgery. We have had a patient fall recently after tripping on the lip of a paving slab so this would be appreciated if it could be looked into urgently. Many thanks
