I have attached a photo of the hole that appeared at the same spot on 23 June last year. It would appear that the last repair hasn't cured the problem. Is there an issue with drainage that is washing the substrata away?

The ground has now fallen in, therefore creating a sink hole in the centre of the road. This will need urgent attention as it’s now putting drivers and public at risk of safety in a highway due to the sink hole being in the centre of the road.
