Very damaged and uneven pavement surface

Large holes in the pavement. I've seen at least two runners trip and fall and an elderly lady has also injured herself walking there. In addition to the abovementioned safety issues it is causing rainwater to drain into my driveway which is causing drainage problems. On top of that this part of the road is also poorly lit, so pedestrians are quite likely to get injured in the dark. It's been ok this state for at least 6 months.
