Pavement defects in Hayes Wood Avenue - odd numbered side of road

There are a very large number of pavement defects in Hayes Wood Avenue. More specifically, these are:
Odd numbered side of road
1. Uneven paving stone outside 5 Hayes Wood Avenue - this needs to be relaid to make sure that it is evenly cemented in place.
2. Several uneven paving stones next to the street tree outside 7 Hayes Wood Avenue - these need to be relaid so they are evenly cemented in place.
3. Loose paving stone outside 17 Hayes Wood Avenue - this needs to be relaid to make sure that it is evenly cemented in place.
4. Tarmacked surface next to the street tree outside 31 Hayes Wood Avenue is cracked and uneven - this needs to be relaid so that it is smooth and even again.
5. Uneven paving stone outside 51 Hayes Wood Avenue - this needs to be relaid so that it lies flat again.
6. Tarmacked surface next to the street tree outside 61 Hayes Wood Avenue is cracked and uneven - this needs to be relaid so that it is smooth and even again.
7. Loose paving stone outside 95 Hayes Wood Avenue - this needs to be relaid so that it lies flat again
8. There are several uneven and loose paving stones beyond the Council's 'trip limit' next to the street tree outside 97-99 Hayes Wood Avenue - these need to be cemented into place so that they lie flat again
9. Uneven paving stone outside 101 Hayes Wood Avenue - this needs to be relaid so that it lies flat again.
Please would you arrange an inspection and then carry out the necessary remedial work as outlined above.
