Water leak from grass verge next to road

There is a water leak outside of my house (584 Bolton Road) on the grass verge next to the road. It appears that ground water makes its way to the surface after a downpour of rain. the water will gush out of the floor after rainfall so much so my neighbours and I thought it was a burst main at first.

A few years back some work was carried out by another party in which they removed the paving slabs from out my house and they re-layed the area with tarmac (not the original paving slabs). The tarmac has begun to subside and now there are significant areas where there is standing water. during the recent cold snap these areas became frozen and are a slipping hazard, especially to my elderly parents.

I suspect that the water leak and the tarmac showing early signs of subsidence are linked. Please can you investigate the leak and rectify the subsidence on the public footpath between the two grass verges ouside of my house.
