Damaged road surface around access cover.

Tarmac around the edge of the access cover is damaged so that the cover moves every time a vehicle passes over it. This causes a loud clattering noise, audible from our home, which is extremely annoying, particularly at night. This is a problem which has occurred previously and the tarmac was repaired. Damage has now been back for several weeks. Location is at the end of the driveway to our home - The Old Cottage.
