Trip Hazzard - Water Main Cover

The water main cover at the top of the photo is ill fitting, I stood on it yesterday and it flipped out of the hole and my foot twisted in it. I laid it back down, but it doesn’t fit properly and I’m worried that an elderly person would get seriously hurt if they did the same.
