Footpath directly outside the gate

The footpath has been deteriorating for years. There is a loss of tarmac and a large concave basin which collects water. People walking along the pavement sometimes have to step into the road to avoid the large puddle. When there is heavy rain the water extends up onto the driveway. My elderly mother lives alone in the property and is at risk for falling on the wet uneven pavement. Please can you fix this. Thank you.
