Loose metal inspection cover and flooding

Sent to Staffordshire County Council 5 minutes later. FixMyStreet ref: 633703.

When it rains floods to half way across carriageway causing a traffic hazard. The loose cover continually rattles as the traffic moves over it, could cause eventual fatigue failure in the metal and obviously another road hazard.
