Concrete breaking down and causing issues

Concrete footpath/access from road to driveways has seriously deteriorated since it was put in (in 1964-ish). In many places the concrete has completely broken up and has huge holes which cause damage to cars entering the driveways across the public highway area (between the road and the driveways). The footpath-only areas are also very rough and deteriorating. I have tripped a couple of times over the past year. This has been slowly getting worse over the last 10-15 years, and is getting dangerous.
