The cover has been like this for a few years now, and I have reported it a few times and nothing had been done, as you can see from the picture there is now a tree starting to grow out of it. It is also very dangerous for people using walking sticks or crutches as the holes are big enough for the sticks to fall down them, it could also cause an injury to either a human or an animal. Is there no any chance could possibly chase up getting it repaired. Thank you very much for your help with this matter.
