General road marking and stud issues problem

I have tried to report this in your on-line portal but the ‘continue’ button does not apple to be active. See below.
The white lines denoting a road crossing have completely disappeared and traffic on the Fosse may not realise that there is a road crossing here. I believe it is a small job as all that is required are the white lines to be repainted. Please could you attend to this urgently.
I am a parish councillor in Shipton Moyne. I have reported this issue multiple times but nothing seems to be happening and I am seriously worried that there may be an accident. Indeed I know of one very close call at this junction.
