Injury from collapsed manhole cover

On the crossing island directly outside North Baddesley village hall I was crossing with my daughter in law and the manhole cover gave way causing her to fall backwards. Had it not been for the traffic being at a standstill due to rush hour this would very likely have caused her to be hit by a car. As you can see from the photo she’s cut her hand and also has pain in her wrist from the fall. This happened this evening at approximately 5.15pm. The fact that’s it’s in a traffic island makes it more dangerous so surely it should be checked regularly for any deterioration. Imagine if that was an elderly person it happened to.
