Pathway dangerous uneven trip hazard

Sent to Leicestershire County Council less than a minute later. FixMyStreet ref: 3969055.

The pathway leading to and from the park is in a very poor state my mum (93) has difficulty negotiating around the uneven surface with her triwalker and pedestrians would quite easily trip especially if dark.
