Oak in decline with decay fungus 'Grifola frondosa' at base

This is a mature Oak on a public footpath. I am an arboricultural consultant who was asked to look at the tree by a neighbouring land owner. The tree has dieback in the canopy and audible decay around the base. There are fungal fruiting bodies around the base of Grifola frondosa . This causes a white rot wood decay and can cause root failure.
