Manhole cover broken/ damaged resulting in loud noise when driven over

Sent to Mid Suffolk District Council and Suffolk County Council less than a minute later. FixMyStreet ref: 3261791.

Every vehicle going out of the village drives over this broken/ damaged manhole, resulting in a loud noise which disturbs the household. Please, please fix it. It is located directly outside our house adjacent to the driveway.
