Metalwork broken leaving dangerous hole

The cover from a service shaft(?) keeps coming out. I've noticed it twice recently and replaced it both times. It seems to be when it's raining. The hole is filled with water each time. I have heard of damage being caused to a vehicle as it drove over the hole, which was barely visible due to the water.
