Concrete missing in pavement causing dip - trips occurred

For at least 1 year there has been concrete broken and removed on the pavement. In periods of rain the hole fills up and passersby cannot tell how deep the hole is. There have been children who have jumped on the 'puddle' and have fallen. Leaves often pile up in the hole meaning people cannot see that its a hole and trip over. a member of staff from a local business has twisted their ankle due to tripping in the hole.
