A number of 40cm potholes in concrete road at end of Lane

I have to constantly report pot holes in the road that have a quick fix and then wash out with the next rains. This has been ongoing for years and I lose track of the number of damaged tyres and wheels I have had - not to mention falling over when walking along at night. This is a council maintained road that needs proper remedial work carrying out before someone really gets hurt.
