Tree roots lifting the pathway/lady fell and hurt herself

At the bottom of Harrison crescent (along the fence)as it meets Delamere road the end house on the left as you are facing Delamere road, the pathway is lifting due to tree roots, my 74 year old mother tripped and fell and grazed her hands and knees and had to be helped up by a neighbour, I have the phots of the grazes she received as a result but it wont let me upload. can someone please come and look/repair the pathway please so this doesn't happen to anyone else.
