Road sunken around gullies - Rutted & cracking

Same area around two gullies on corner has sunken badly over the summer.
This patch has never been put back right since British Gas dug it up years ago & reinstated it badly.
This was investigated back in Feb (985685) But deemed not bad enough for intervention at the time. Since then the condition has worsened considerably.
On top of the carriageway issues I'm fairly sure one of the gullies just fills with water during rain while the other is actually now above the road surface so, pretty useless.
Would appreciate it if someone could have another look.

I have visited this location on 03.10.2019. Upon my inspection I could not find defects that would meet safety levels for intervention.
Upper Hale road is subjected to monthly inspections under Scheduled Highways Safety/Condition Inspections of the Surrey Highway Network. From the information provided by these inspections the operations team evaluate whether a road/footpath will be re-surfaced or not.

I have inspected two gullies at pinpointed location. Both looked operational with visible outlets. Smaller gully was clean and in good condition the other larger gully had some debris at the bottom but outlet was still visible and operational. I will monitor this location and add this gullies to the list for investigation if required.
