Protruding manhole cover at the junction of Richmond Road and Queens Road. Trip hazard causing personal injury.

The rectangular manhole illustrated has significantly protruding edges and corners as show, causing me to trip and fall heavily on my left knee (photograph also attached). Please repair this to avoid injury to others and advise the procedure for claiming compensation for injuries, loss of earnings and damage to personal effects.
