Drain cover surround loose within deep and wide pothole

The drain cover has previously been repaired but the wider section has again failed due to continually high HGV traffic. There is a 6 inch wide deep hole surrounding this drain cover, which is a danger to tyres, wheels and cyclist as this can be covered by standing water when the drain doesn't do its job.
The hole was repaired within 48 hours of the previous report in February - by the dumping of two buckets of tarmac on the drain, thereby covering the hole - and the drain itself. The repair has of course failed as the tarmac has now been pushed through the drain by the heavy HGV usage. And is now in just as bad a condition as it was before. Even more so now, as puddles sit in the hole, making it very dangerous when wet.
