Large access manhole dropped below surface of road

The large access manhole right outside the fire station has dropped below the surface of the road, this has been outstanding for several months now. It has been levelled off twice with tarmac but this just breaks up with all the traffic moving over it, it then starts to bounce up and hit the underside of the cars .There is only half if it there now as it has been broken up and washed away.
