Tree causing raised pavement again

This problem was allegedly cured a few years ago. I was told the roots would be cut and it wouldn’t happen again. It has, as you can see it has raised up again (easy to trip over) and the surface that was put down has spreading cracks, one right up to my garden wall and others spreading towards it. It is getting worse and I don’t want my wall damaged or someone injuring themselves on a problem that was not sorted out properly.
