Broken paving slab in front of Sainsbury's local 31B Clifton Road, Rugby CV21 3PY

Reported anonymously at 14:25, Mon 17 February 2020

Accident 14.2.2020 There is a broken paving slab in front of the store which caused me to trip and fall forward into the iron roof support and iron downpipe of the shop, I lost consciousness briefly, sustaining injury to my shoulder blade, neck, left arm, right hand, and both knees. I attended A&E at Hospital of St Cross and UCHW Coventry where I received examinations and advice, I felt very unwell and attended St Cross again on 16th for further advice. Other photographs are available. NB: The manager of Sainsbury has cctv footage of the accident and has promised a cd would be made, and be available if necessary. I was unable to see it because of DATA Protection .
