Loose manhole cover

Square metal manhole/access cover is loose and rattles everytime a vehicle drives over it.
It is on the junction of City Road and Dingley Road on the north-west bound carriageway.
It was fine until a couple of months ago. We're guessing some work was done and the covers were not replaced in the correct order (the white road markings on them do not match the markings on the tarmac around them).
Because City Road is a busy road it means that residents of the buildings around this problem are constantly disturbed by the noise.

Updates
Contacted the Islington Council Noise Team directly who responded within 3hrs and notified TfL who maintain the road. Problem was fixed the same weekend. Very impressed with the swift response although very annoyed that fixmystreet.com didn't make it clearer the council should be contacted directly for noise pollution issues (it's hidden in the FAQs).
